How to Register Your Student
Registration forms are available here or at any of our school offices. Military families may also obtain forms directly from the School Liaison. If you prefer to receive the registration packet via email, please send your request to data@carthagecsd.org.
Required Documentation
All necessary instructions and documents are listed in the welcome letter under the required documentation section. The following must be submitted with your completed form:
- Proof of residence
- Birth certificate (or other approved documentation as outlined in the welcome letter under required documentation section)
- Custody paperwork (if applicable)
- Current NYS physical and proof of immunizations (Must be provided by the start of school. Detailed requirements can be found on our Health Services page.)
Submitting Your Application
You may return your completed packet and supporting documents via:

- In-Person: Drop off at any school office.
What Happens Next?
Once processed, the individual school building your student will attend will contact you regarding teacher assignments, transportation, orientation, and other start-of-school details.
