Sport Sign Up Procedure

Students in grade 7-12 wishing to play a middle school or high school sport, must register online at

Sports Physicals

Contact the school nurse to see if your child needs a sports physical.

High School Nurse- Amanda Terrillion - 315-493-5030

The Carthage Central School District will accept sports physicals from NYS certified physicians or from the School-Based Health Clinic.

-If physicals are done off campus, be sure to submit a copy of the physical to the school nurse or Athletic Office - not to the coach.

-IT MUST BE RETURNED AND REVIEWED BEFORE A STUDENT IS ELIGIBLE FOR INTERSCHOLASTIC ATHLETICS.
