School Based Health Clinic  

Summer 2018 Operation

Carthage Area Hospital will have extended summer clinic operation hours for 2018. The clinic will be held in the regular Carthage High School Clinic. This clinic will be open to all students in the Carthage Middle, Carthage High and Beaver River Schools.


The Clinic will operate with a LPN and PA onsite in both July and August. The clinic will be open every Tuesday and Wednesday from 9a-2p starting the week of July 15, 2018 and run through August, until school starts.


The clinic will be enrolling new students to the clinic, offering mandatory immunizations for school, physicals for school and sports as well as regular sick visits. Appointments are recommended for physicals.


What does it cost to use the SBHC over the summer?

You will NOT pay for:

·         Any out-of-pocket costs for a health care visit at the school. Treatment for illness and minor injury, follow-up visits and check-ups are all provided at no out-of-pocket cost to you. The same is true for behavioral health counseling and nutrition sessions.
We will bill your insurance; however there is no out of pocket expense!

Depending on your insurance coverage, you MAY need to pay for:

·         Lab tests and x-rays that are done outside of the SBHC (blood work, lab cultures and strep tests).

·         Consults with specialists and any other specialty care.

Revisions to School Health Services

Regulations Effective 7/1/18

Effective 7/1/18, the mandated years for health exams and screening will change.
In addition, only the approved NYS School Health Examination Form may be used to document health exams to provide consistency across the state. Please find this form in the Document Container below and on the School Health Forms or School Health Appraisals page.

The Changes Are Provided Below:

  • Health examinations will be required in grades PreK or K, 1, 3, 5, 7, 9, and 11.
  • Vision screening for distance and near vision acuity will be required of admission to school and in grades Pre K or K, 1, 3, 5, 7, and 11.
  • Hearing screening utilizing pure tone testing will be required of admission to school and in grades Pre K or K, 1, 3, 5, 7, and 11.
  • Scoliosis screening will be required in grades 5 and 7 for girls and grade 9 for boys.
